Fica declaration form (less than 3 months old). This form can be obtained here or from any Capitec Bank branch and must be completed by the declarant. The declarant must include an original address verification document issued in his/her name that matches the acceptable documents listed here. |

Tribal village/Traditional authority letter (less than 3 months old, only if you're employed full-time or part-time). This letter, with your salary slip as a supporting document, can only be used if no other document is available. The letter must contain your name and surname, the name of the designated official (tribal chief or leader), in terms of what legislation, ordinance or regulation authority was assigned to the designated official, confirmation by the designated official that you live on the communal property under control of the traditional authority, the date when the letter was issued, and the signature and stamp of the designated official. |

Employer confirmation letter (less than 3 months old). This letter can only be used if no other document is available. It must be on an official company letterhead with your name, surname, ID number, confirmation of employment and residential address in accordance with your employer's records, and must bear the signature of your employer. |

Councillor letter (less than 3 months old, only if you're employed full-time or part-time). This letter, with your salary slip as a supporting document, can only be used if no other document is available. The letter must be on the municipality's letterhead and must contain your name, surname, ID number and residential address, the date when the letter was issued, an official stamp of the municipality, and the name and signature of the councillor who completed and certified the letter. |

Electronic statement (less than 3 months old). This document can only be used if no other document is available. Emailed statements for accounts such as municipal rates and taxes (utilities), cellphone and retail stores must be printed and may be accepted as proof of residential address. The electronic statement must be on the official letterhead or bear the recognisable logo and information of the organisation, and it must contain your full name or initials, surname and residential address. This statement may not be emailed to the branch. |

Don't have proof of residential address? If you are a South African citizen and an employed individual, pensioner or student, please download and print this Fica declaration form. Complete it with your landlord, home owner, college residence manager, hostel manager or cohabitant. Then bring it and the documents listed above to the branch to open your account. Remember that if the declarer is your cohabitant, they must also provide proof of their residential address, except where he or she is an existing Capitec Bank client. It is not a requirement for the declarer to accompany the client to the branch but the client must obtain a copy of the ID of the declarer. If the declarer is an existing Capitec Bank client, it is not necessary to obtain the proof of residential address and ID of the declarer.
